Does contact buddy allow you to change the labels to Spanish? If so, how?
Is it just clicking a checkbox? or do I need to hack code?

You are going to need to hack some code but we will be happy to answer an questions you have.
Here are the line numbers of the labels you will need to change:
<p style="padding-left: 30px">contactbuddy.php:
<p style="padding-left: 60px">"Name:" is on line 171.
"Email:" is on line 177.
"Subject:" is on line 183.
"Message:" is on line 189.
