Cimy User Extra Fields With Exchange

Since Exchange cannot record address/phone number information or display billing information, I am forced to find yet another plugin to fill the gap.
I have downloaded and installed Cimy User Extra Fields plugin. It is configured correctly but how do I get Exchange to display the extra fields in the registration form?

I have already set Exchange to use Wordpress Registration Setting for Customer Registration.

PS. I also set Allow Anyone to Register in the Wordpress General settings.
